House calls for those unable to make it out to the surgery whether due to immobility, disability, frailty, postoperative recovery, post hospital discharge -stepdown home based care, pain management, end of life care, post injury, muscular skeletal issues, bedridden condition, poor social support, transport issues etc
Telephonic consults
Telephonic consults are Ideal for common ailments, chronic conditions, repeat prescriptions, ongoing care and follow ups etc. These take place either from your home /work/anywhere etc or sometimes with the patient at a pharmacy/community nurse will enable a basic initial assessment by having simple observations remotely such as blood pressure to aide your care when possible.
